Aviva Health’s COVID-19 vaccination site is open weekdays.

Its located just across the street from the main Aviva clinic location on Northeast Stephens near Costco. The clinic offers free COVID-19 vaccinations by appointment to those 12 years of age and older. Residents 12 to 14 must have parental or guardian consent to be vaccinated. Written consent can be obtained in advance. Under Oregon law, minors 15 years of age and older can consent medical treatment including vaccinations.

Hours are 8:00 a.m.to 5:00 p.m. Call 672-9596 to make an appointment.
