The Aviva Health dedicated COVID-19 vaccination clinic site remains open on weekdays.

The clinic is located at 4221 Northeast Stephens Street, across the street from the main Aviva Roseburg clinic. Hours are 8:00 a.m. to 5:00 p.m. The clinic is open for those age 12 and older. Parental or guardian consent is required to vaccinate those age 12 to 14. Written consent can be obtained in advance. Under Oregon law, minors age 15 and older can consent to medical treatment, including vaccinations.
