Adapt Integrated Health Care has released a notice of a security incident and potential breach of information.
An Adapt release said the information is being provided regarding a data security incident that occurred at TriZetto, a third-party company that works with Adapt’s electronic medical record system provider, OCHIN.
Adapt learned of this incident on December 10th, when OCHIN notified Adapt that an unauthorized individual had gained access to one of TriZetto’s systems. TriZetto took steps to stop the unauthorized activity and secure its systems. After learning that the incident may have impacted some Adapt patients, Adapt began working with OCHIN to understand what occurred and to help protect patient information.
Adapt’s internal systems were not breached. Not every patient’s information was affected. Beginning in February, TriZetto plans to send individual notification letters to patients whose information may have been involved.
Patents are encouraged to remain alert for suspicious activity, such as unexpected bills, insurance statements, or communications requesting personal information. Anyone who notices unusual activity should contact their health insurer and/or bank or financial institution immediately.
TriZetto has established a dedicated toll-free call center with information at 844-572-2724. Individual patient notices will include information about pre-paid credit monitoring services and instructions on how to enroll.
