Adapt Integrated Health Care has achieved the esteemed 5 STAR Patient-Centered Primary Care Home designation.
An Adapt release said it is the highest recognition awarded by the Oregon Health Authority.
The honor distinguishes Adapt as a leader in implementing advanced, transformative, processes that deliver exemplary care to its patients and the broader community. The 5 STAR designation is reserved for clinics that excel in meeting and exceeding rigorous standards set by the PCPCH model framework.
Adapt Director of Operations Robin Peterman said, “Earning this award is a strong accomplishment dedicated by each and every staff member who plays an essential role in Primary Care”. Peterman said, “I am so proud of this team’s work and passion to serve exemplary care for our patients and community”.
The release said Adapt’s recognition as a 5 STAR PCPCH reflects its leadership in setting a new standard for primary care delivery. By implementing innovative workflows and fostering collaborative relationships with patients and families, Adapt has created a model of care that is accessible, accountable, and transformative, according to the release.
