A crash on Highway 138W claimed the life of a Salem man on Thursday afternoon.

A report from the Oregon State Police said a preliminary investigation indicates just after 12:35 p.m. a box truck operated by 68-year old Jose Hurtado was eastbound about five miles west of Sutherlin when for unknown reasons, it went across the roadway and partially onto the westbound shoulder. The truck veered back across the roadway and onto the eastbound shoulder where it crashed into a tree. Hurtado was pronounced deceased at the scene by first responders.

The highway remained open during the on-the-scene crash investigation.

OSP was assisted by several other agencies.
