Staff with the Boys & Girls Club of the Umpqua Valley are hosting a Warm up to Wellness event on January 11th.
The Club is partnering with Evergreen Family Medicine and the Oregon Department of Human Services for the Saturday event which is for children up to age 18.
Evergreen Family Medicine will provide Well Child checks, sports physicals, health screenings, and vaccinations at no charge to the patient. Insurance information should be provided if applicable.
The event will also include carnival games, activity booths, snacks and prizes for all ages. Wildlife Safari will bring exotic animals as well. It will run from 9:00 a.m. to 3:00 p.m.
A parent or guardian consent for medical services is required for children under the age of 15.
